What is OSF MyChart?

OSF MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With OSF MyChart, you can use the Internet to:

  • Request medical appointments.
  • View your health summary from the OSF MyChart electronic health record.
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Communicate electronically and securely with your medical care team.

What is OSF OnCall?

OSF OnCall is a virtual urgent care clinic that allows patients to access a provider anytime of the day or night, using a smartphone, computer or tablet. Providers can diagnose and treat a number of minor illnesses and even prescribe medication, if appropriate. Learn more at www.osfoncall.org.

How do I sign up?

Patients who wish to sign up for an OSF MyChart account can sign up online by clicking on the "Sign Up Now" button.

Is my access code my user ID or password?

No, your access code is not your OSF MyChart ID or password. You will use this code only once to log into OSF MyChart for the first time. The access code will expire after you have used it to enroll or after 45 days. When you log into OSF MyChart the first time, you will be asked to create your own unique OSF MyChart ID and password.

My activation code does not work. What should I do?

For your security, your activation code expires after 45 days and is no longer valid after the first time you use it. You may call our OSF MyChart Call Center at 855-OSF-HEALTH (855-673-4325) for assistance.

What is your Privacy Policy?

OSF MyChart is owned and operated by OSF HealthCare and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by OSF MyChart. Review our privacy policy in detail.

When can I see test results and clinical notes in OSF MyChart?

Your test results and any clinical notes are released to your OSF MyChart account as soon as they are finalized. You may see your test results at the same time or before your provider is able to contact you with results. There may be instances your provider chooses to withhold a result until he or she can discuss it with you. You can still expect provider follow up, as appropriate.

Tests prohibited by state law are not automatically shared to OSF MyChart. Further, your provider is able to determine which types of test results are able to be accessed through OSF MyChart. There are two exceptions in which test results and clinical notes may not be shared:

  • If the provider feels that the information would cause physical harm to the patient or others
  • If the patient requests the information be excluded from OSF MyChart

What portions of my medical record can be seen within OSF MyChart?

Most interactions with an OSF HealthCare facility or provider will be documented as a clinical note. This includes hospital or outpatient visits, telephone conversations and other personal health information which may be considered sensitive in nature. Examples include:

  • Lab results such as complete blood count, hemoglobin A1C
  • Imaging results such as CT scans and X-rays
  • Clinical notes related to your care
  • Referrals to specialists
  • Social history (use of tobacco, alcohol or drugs, sexual activity)
  • Sexual orientation and gender identification

If some of my health information on OSF MyChart is not correct, what should I do?

Your OSF MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next office visit. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to someone via OSF MyChart, when can I expect a reply?

You will generally receive an answer by the end of the next business day. OSF MyChart is NOT to be used for urgent needs. For medical emergencies, please dial 911 or go to the nearest emergency room.

Can I download a copy of my medical record?

Yes, you can download a copy of a portion of your medical record. Once you have signed up, please see "Share my Record" under the "Sharing" menu for more information.

Can I update my allergies and medications?

Yes, you can request updates to both your allergies and medications. Allergies are found in "Health Summary" under the "My Record" menu. Medications are found under the "My Record" menu. If you request an update to your medications or allergies, your doctor's office will review your request during your next visit before making the change in your health record.

Can I view my hospital visit information?

Yes. After you are discharged, or upon completion of your visit, you will be able to view some information from your hospital visit, including test results and your discharge instructions.

Can I view my upcoming tests and procedures?

Yes, you can view your upcoming tests and procedures by clicking on "Upcoming Tests and Procedures" under the "My Record" menu. You'll be able to view all of your lab and procedure orders, along with the expected date, expiration date, order comments, and number of tests remaining - if applicable.

Can I view a family member's health record in OSF MyChart?

Yes, you can. This is called proxy access and allows a parent (or legal guardian) to log into their personal OSF MyChart account and connect to information regarding their family member.

Individuals with proxy access will be able to view the same personal health information that you are able to access within your own individual account. Because your medical record contains information that may be sensitive in nature, we recommend that you periodically review who has access to your account.

Child proxy access is available, as well as adult-to-adult proxy access. Once you log in, please visit "Request Family Access" under the "Communication" menu to add or revoke access.

Can I ask questions regarding a family member from my OSF MyChart account?

OSF MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care. You can, however, use proxy access (see above) to access that family member's account and send a message on their behalf.

No. Due to the sensitive nature of medical information, each adult must establish their own MyChart account. You can, however, use proxy access to view another adult's information. Please visit "Request Family Access" under the "Communication" menu for more information about proxy access.

Log into OSF MyChart to access "Communication Preferences" under the "Account Settings" menu. Text messages, e-mails, and mobile app notifications are all available.

How is OSF MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Further, OSF MyChart uses the latest in industry-standard 128-bit SSL encryption technology with no caching to automatically encrypt your session with OSF MyChart. Unlike conventional e-mail, all OSF MyChart messaging is done while you are securely logged on to our website.

OSF MyChart also offers 2-factor authentication as an extra layer of security. Access "Security Settings" under the "Account Settings" menu to set up 2-factor authentication. For more information, check out our helpful video on two-step verification.

You might receive email or text notifications that contain links to OSF MyChart. These notifications allow you to login to OSF MyChart securely using a link that expires after a short time. Keeping your OSF MyChart account secure means keeping your phone number and email account secure. If you think that your phone number or email account may be compromised, you can change the phone number or email account on file on the "Personal Information" page in OSF MyChart.

I forgot my username or password. What should I do?

You may contact our OSF MyChart Call Center at 855-OSF-HEALTH (855-673-4325) to request a new, secure password, as well as receive assistance with your username. You may also use the "Forgot Username?" and "Forgot Password?" links on the login page to reset your username or password.

I was logged out of OSF MyChart. What happened?

We aim to protect your privacy and security of your information. While logged into OSF MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of OSF MyChart. We recommend that you log out of your OSF MyChart session if you need to leave your computer for even a short period of time.
