          Can my spouse and I share one MyChart account?

          No. We are committed to protecting the privacy and confidentiality of each individual's health information. For that reason, each adult must have their own MyChart account. However, members of the same family can grant proxy access to their account from within MyChart.

          Is information in MyChart confidential? Will it be shared with my employer?

          MyChart is a secure portal that makes it easy and convenient for patients to access the health information contained in their medical record. Your MyChart account information is not accessible to your employer.

          Can I access Wellness Online through MyChart?

          Yes, once you login to MyChart, click "Health" on the top menu bar, and choose "Wellness Online Portal.”

          How do I sign up?

          Visit mychart.myquadmedical.com and click "SIGN UP NOW". Patients may also refer to a recent patient statement for a MyChart activation code or request a MyChart activation code during their next visit to the health center. The code is generated just for you to login and create your own user ID and password.

          If I already have a MyChart account with a different health system, do I need to create a separate account for QuadMed?

          Yes, you will need to create a separate MyChart account for QuadMed. Once your QuadMed account is created, MyChart will recognize if you have an account at another provider organization and will prompt you to link your respective MyChart accounts if you wish.
          Some features, such as scheduling, provider messaging and viewing test results, are organization-specific. To access these functions, you will need to login to your MyChart account at the respective provider organization.

          Can I access information from QuadMed's previous patient portal?

          Many of your records will be available in MyChart. Some information including past secure messages with your provider will not transfer over to MyChart. If you're looking for medical records but don't see them in your MyChart account, you may request a copy of your records by visiting myquadmedical.com/content/medical-records and completing the request form.

          When can I see my test results in MyChart?

          Most test results will be released to your MyChart account 24 to 72 hours after the final results. Certain test results are not released to your MyChart account until your provider has reviewed them.

          Why are certain test results not shared in MyChart?

          Test results of a very sensitive nature are not released to MyChart. This allows your provider to deliver the results with the proper personal communication and explanation.

          If there is incorrect information in my MyChart account, what should I do?

          MyChart displays information from QuadMed's electronic medical record. If you feel your information is incorrect, please contact your health center or discuss with your provider directly during your next visit.

          Can I view a family member's health record in MyChart?

          Yes, this is called proxy access. Proxy access allows a patient to login to their personal MyChart account, and then connect to information regarding a family member. Proxy access can be requested in the health center or through MyChart.

          Can I access my child’s health information in MyChart?

          Yes, your child’s health care can be easily managed through your own MyChart account with convenient proxy access. Due to patient confidentiality laws in certain states, the type of proxy access is determined based on the age of the child.

          Ages 0-11

          Parents or legal guardians will be granted full access to the patient’s MyChart record, including scheduling and messaging.

          Ages 12-17

          Parents or legal guardians will be granted scheduling and messaging access to the patient’s MyChart record. Proxy access will no longer include the clinical features of the patient’s MyChart record.

          Age 18 & Older

          At age 18, parents or legal guardians no longer have access to their dependent’s MyChart record. Once the child reaches age 18, they must create their own MyChart user ID and password. They will then have the ability to grant proxy access to family members by sending a proxy invitation from their own MyChart account.
